What you need to know about your health risk assessment.
The assessment will ask you about:

  • Your Height & Weight
  • General Health Questions, Chronic Diseases, Life Style & Eating Habits
  • Your Last Blood Pressure Reading
  • Your Last Cholesterol
  • 

If you do not have this information, your online assessment may not reflect an accurate evaluation of your health status. These 'numbers' could be known by your doctor's office if you have had any recent lab work done as part of an annual physical.

You will need your personal logon to proceed and enter the online Health Risk Assessment. This logon will be sent to you and at the first logon you will be asked to change your personal password.

 Having these "real" numbers provides the foundation for positive health choices. Biometric screenings can include a complete blood profile, blood pressure, and a resting and exercise ECG. These are essential tools used to provide this important information.
