| When participating in a Health Risk Assessment, you will need to know your Height & Weight; your last Blood Pressure reading and your Cholesterol levels. This is an online assessment and your unique logon will be sent to you. |
| You may elect to do a 'one on one' biometrics for your Health Risk Assessment. Arrangements for this 'one on one' assessment can be made by your employer contacting Beth Fisher at the Benefits Connection office - 302-294-2059. |
